The Jamaica immigration form also known as the Immigration/Customs C5 Card is a required document that all travelers need to complete before entering Jamaica. This form is necessary to get information about you, and your purpose for visiting Jamaica.

You can access the digital declaration form here. This user-friendly form can be filled out online up to 30 days before your scheduled arrival date, streamlining the entry process and reducing congestion at Jamaica's immigration points.

There is no cost whatsoever to either access the form, complete the form, or submit the form.
