What is Arthur Shelby's mental illness?
He suffers from ptsd and mental instability. During childhood, he was probably ignored by his father and received severe neglect. He grew up with gangsters and cut throats, probably always feared in school.
From six of psychological disorder who described by Cherry (2019) there are just three which Arthur has schizophrenia/delusional, trauma and dissociative disorder. Cherry (2019) claimed the symptoms of schizophrenia was Delusions, beliefs that conflict with reality.
Tommy has effectively taken over the Peaky Blinders, and Arthur feels like he is losing his sense of purpose. Personality… crude, violent, and depressed. Arthur is spirited and lively, but deep down, he is battling serious demons.

Personality Disorder, specifically, Histrionic Personality Disorder plays a key part in Harley Quinn's life. People with Histrionic Personality Disorder are “pervasive and excessive emotionally and display attention-seeking behavior” (Bornstein 1998).
Tommy Shelby, the gang's leader, along with his siblings, Arthur, John, Ada, and Finn, have Irish-Romani heritage on both sides and consider themselves Gypsy. Their paternal aunt, Helen McCrory's irreplaceable Polly Gray, is the daughter of “Gypsy Princess” Birdie Boswell, and their mother comes from the Lee family.
Sometime between the years of 1922 and 1924, Arthur meets religious Linda, a devoted Quaker, and they marry. She helps Arthur put his life back together and deal with his mental health issues, firstly by helping him stay sober as much as she can.
Linda was a very devout Christian when Arthur met her, and at the start of their marriage she seemed able to keep him on an even keel. But living with the Peaky Blinders has finally got to her. She has become unpredictable, angry, and is heading towards a showdown with her husband.

One of the major symptoms of PTSD is "recurrent, unwanted distressing memories or dreams of the traumatic event."[1] A number of characters in Peaky Blinders appear to suffer from it, the most notable ones being Thomas Shelby, Arthur Shelby Jr., and Danny Whizz-Bang.
He had tuberculoma in his brain stem, a growth caused by the same bacteria as the lung disease, doubtless picked up from daughter Ruby (Orla McDonagh). It wasn't infectious but it was inoperable. The symptoms (seizures, dissociation, hallucinations, sharp cheekbones – I may have added the last one) chimed with Tommy's.
He is about to pull the trigger when he hears his late daughter's voice. Tommy then sees and talks to the ghost. The supernatural has been discussed in the show as their family are gypsies and believes (especially in this final season) in ghosts and some superstitions. She is the one that tells him that he is not sick.
As a Quaker, Linda is incredibly devout and is extremely disapproving of Arthur's participation in the illegal side of the family business. Linda is skilled at navigating the intense and volatile personalities of the people around her.
SPOILERS: When John visits Arthur in jail he mentions arthur tried to hang himself twice.... but... that only happened once.
In Peaky Blinders, Tommy Shelby and Polly Gray were seen half naked dressing each other up, presumably after sleeping together.
It's no secret that Arthur Morgan didn't have the best family life. His mother tragically passed away when he was young, and his father failed as a single parent. Rather than supporting and loving Arthur, his father remained emotionally distant, committed various crimes, and was eventually arrested for larceny.
In Joker, Thomas Wayne (Batman's father) and Penny Fleck (Joker's mother) are key characters. Penny claims that Thomas is actually Arthur's father.
If Arthur chooses to help Mary, their relationship improves to the point in which she reveals she still has feelings for Arthur. She asks if its too late for them to have a future together and asks if they could run away together.
The Joker gradually seduces Quinzel and gets her to fall madly in love with him. He later persuades her to release him, subjecting her to shock therapy in retaliation for her using it on him, and manipulating her into jumping into a vat of the same chemicals that disfigured him to prove her love for him.

Individuals with BPD can use sex and be known for promiscuity, but the level to which Harley excessively exhibits these traits is more indicative of Histrionic Personality Disorder. Many consider HPD to be a subtype of BPD.
But at night, his anxiety runs rampant. So he “self-medicates” by smoking brown opium. His brother Arthur also has a predilection for drugs and favors cocaine. Arthur's “use of the drug is more than just an aspect of his character.
Cillian Murphy also said he married her for loyalty. By s4, she was not a prostitute but a secretary-cum- friend with benefits. She is low maintenance, he had no emotional hassle with her and basically did what was told. So its a marriage of convenience overall.
Tommy and others are seen smoking Opium throughout the series as a coping mechanism. Over 5000 cigarettes were smoked on the show. Characters drink frequently on-screen, become drunk, and act violent and clumsy. One character has an opium habit, and a 10-year-old child smokes a cigarette on-screen.
