Your subscription is connected to your Apple ID. It's not possible to transfer the subscription to another Apple ID. If you want to use your subscription on another iOS device, you need to log into your Apple ID in the device settings.
You cannot designate a specific card for a specific subscription. You would have to change the primary card prior to the item being charged, and then change it back again.
Click your name at the bottom of the sidebar, then choose View My Account. You may need to Sign In with your Apple ID first. Scroll to Subscriptions, then click Manage. Next to the subscription you want to change, click Edit.
On your new iPhone, go to Settings > Cellular, tap Set Up Cellular or Add eSIM, then tap Transfer From Nearby iPhone or choose a phone number. On your previous iPhone, follow the instructions to confirm the transfer.
